Hello everyone.. I am varshitha , an intern posted in medicine department and one of the important terms of getting the internship completion is to complete my log book with my online log of what I learn during the course of my duties.


Here we discuss our individual patients problems through series of inputs from available global online community of experts with an aim to solve those patients clinical problems with collective current best evidence based inputs.

Here is the case I have seen-


A 61 year old female came to the opd with the chief complaint of fever since 6 days ,headache since 6 days ,body pains since 6 days, vomiting since morning (3/09/2020) 


 History of presenting illness-

Patient was apparently asymptomatic  6 days back,then she developed fever which was insidious in onset,lowgrade, intermittent type ,No h/o night sweats .Patient gave H/o fever since one month for which she went near a Rmp and took medications. She gave h/o headache since 6 days for which she went near an ophthalmologist and got specatacles for the same  ,generalized body pains since 6 days,h/o giddiness since 6 days ,no h/o diplopia,blurring of vision,cough,weight loss,burningmicturition,constipation,shortness of breath,malena,heametemesis,loss of appetite


Past history -

Not a k/c/ o DM,HTN, thyroid , epilepsy , asthma

No past surgical history 


Personal history-

She has a mixed diet, with normal appetite ,adequate sleep and regular bowel and bladder movements


Family history-

Not significant


General examination -

Patient was c/c/c , moderately built and nourished 


P/A-

Shape- scaphoid, no local rise in temperature, no tenderness, no organomegally, hernial orifices are free


Cvs- 

S1 S2 heard, no murmurs


R/S- 

Normal vesicular breath sounds heard, Trachea - midline, no wheeze, no adventitious sounds 


CNS-

Higher mental functions normal

Motor system- intact

Sensory system - intact

Cranial nerve examination- normal

DIAGNOSIS -
PANCYTOPENIA UNDER EVALUATION 
? MEGALOBLASTIC ANEAMIA 


Treatment-

Inj Pantop 40 mg IV BD

Inj Piptaz 4.5 gm IV TID

Tab Pcm 500 mg sos

Inj vitcofol 1000 ug  IM alaternate day for 5 days
